    All medical insurance claims associated with therapy sessions are the responsibility of the individual.
 Payment is due at time of the therapy session. It is the responsibility of your insurance provider to reimburse you.

   Receipts are available upon request. If you have a "Flex-Spend" account included in your insurance plan, you will be re-imbursed when you present your receipt.
